What Is Interobserver Agreement

A method for improving data credibility by comparing the independent observations of two or more people in the same event. The IOA is calculated by taking the number of agreements between independent observers and dividing them by the total number of agreements plus disagreements. The coefficient is then multiplied by 100 to calculate the percentage (%) of the match. The evaluation of agreement between observers is fundamental to the quality of data in time and motion studies. Cohen`s Kappa study should not be considered the gold standard for agreement in time and motion studies. The Inter-Observer Agreement (IOA) is a key aspect of data quality in clinical work time and movement studies. So far, these studies have used simple, ad hoc approaches to IOA assessment, often with minimal reporting on methodological details. The main methodological problems are how to align task intervals with timestamps that rarely have corresponding start and end times, and how to evaluate the IOA for multiple dummy variables. We present a combination of methods that address both problems simultaneously and provide a more appropriate measure to assess IOA for time and motion studies. The problem of alignment is solved by converting task-level data into small time windows and then aligning data from different observers by time. A method applicable to multivariate nominal data, the Iota score, is then applied to timed data.

We illustrate our approach by comparing Iota scores with the average Cohen-Kappa univariate scores by applying these measures to existing data from an observational study of emergency physicians. Although both results yielded very similar results under certain conditions, iota was more resistant to scattered data problems. Our results suggest that Iota applied to time windows significantly improves previous methods used for IOA assessment in time and motion studies, and that Cohen`s kappa and other univariate measurements should not be considered the gold standard. Rather, there is an urgent need to continuously and explicitly discuss methodological issues and solutions to improve how data quality is assessed in time and movement studies to ensure that the conclusions drawn from these studies are sound.
